Abstract

Postoperative cognitive dysfunction (POCD) occurs immediately after surgery and is characterized by impairment of memory and changes in cognition. POCD can last for several months or years and have adverse effects including delayed hospital stays, diminished function in daily life, and increased complications and mortality. Despite improvements in surgical technique, anesthesia management, and intensive care, many patients suffer from POCD. POCD is one of the important clinical issues in surgical management and understanding its pathophysiology is necessary. In this review, therefore, we have focused on animal models of POCD and measurements of cognitive ability in preclinical studies, and we have suggested novel approaches for prevention/treatment of POCD. In preclinical studies, major abdominal surgery (laparotomy, hepatectomy, and splenectomy), minor abdominal surgery (laparotomy, probe exploration), and tibial fracture surgery, are used as POCD models. In addition, cognitive function is assessed by Morris water maze, passive avoidance task, elevated plus maze, and T maze test. Neuroinflammation, blood-brain barrier dysfunction, beta amyloid deposition, and tau phosphorylation are suggested as pathological mechanisms of POCD in preclinical studies. Based on several studies of these, we suggest erythropoietin, nuclear factor kappa B, interleukin17A, tumor necrosis factor alpha, and nicotinamide adenine dinucleotide phosphate oxidase 2 as candidates for prevention/treatment of POCD. In the preclinical stage, drug development/exploration and research is being carried out to solve cognitive dysfunction after surgery. Ultimately, based on the results of preclinical studies, we expect to overcome POCD.

서론

수술 후 인지 기능 장애(postoperative cognitive dysfunc춗ion, POCD)는 수술 직후에 발생하며, 신경 심리 검사를 통해 평가되는 인지능력의 변화로 기억 및 사고의 손상이 특징이다[1,2]. 이는 수개월, 수년 동안 지속될 수 있으며, 수술 후 재원 기간 연장, 일상생활의 기능감소, 합병증 발생과 사망률 증가 등의 부적정인 영향을 미칠 수도 있다[2-4]. 특히, 수술 후 인지 기능 장애는 노인환자에서 호발되는 것으로 보고되고 있고 나이가 중요 위험인자로 여겨지고 있지만 다른 연령대 환자도 예외는 아니다[2,5]. 이전 연구결과에 따르면, 60세 이상의 고령에서 POCD의 발병률은 1주일 후 25%, 3개월 후 9%의 비율로 나타난 것으로 보고되고 있으며 중년 환자(40세–60세)에서는 수술 후 1주일 후 19.2%, 3개월 후 6%의 POCD 발병률을 보였다[2,5]. 또한, 18세에서 39세의 젊은 환자에서도 높은 유병률을 보여 모든 연령대에서 인지 기능 장애의 위험요소를 가지고 있었다[3]. 수술 기법 발전, 마취 관리 및 집중 치료의 향상에도 불구하고 많은 환자들이 수술 후 인지 기능 장애를 겪고 있으며[1], 병인으로 마취 시간, 수술 후 감염, 호흡기 합병증, 나이 등이 제시되었지만 수술 후 인지 기능 장애의 정확한 원인과 기전에 대해서는 잘 알려진 바가 없다[2]. 수술 후 인지 기능 장애의 극복은 중요한 임상적 문제 중 하나이며 먼저 그 기전/병태 생리학을 이해하는 것이 고려되어야 한다. 그러므로 저자들은 본 리뷰에서 전임상 연구에서 수술 후 인지 기능 장애의 재현 모델과 인지기능 측정방법을 알아보았고 또한 이를 통하여 밝혀낸 병적 기전과 위험요인 및 치료와 예방을 위한 후보물질들을 제시하고자 한다. 이를 통하여 효과적인 수술 후 인지 기능 장애의 예방과 치료에 도움을 주고자 한다.

전임상에서 수술 후 인지 기능 장애 모델

Major abdominal surgery model

임상에서 노인 환자의 복부 수술 이후 인지 기능 장애 및 섬망 발생률이 35%–60%로 나타난다고 보고 되고 있다[6]. 실제 전임상에서 수술 후 인지 기능 장애 모델은 복부 수술을 재현한 모델이 많았다. 임상에서의 수술을 재현하기 위해서 Institute of Cancer Research (ICR) 마우스를 3% sevoflurane으로 마취시키고 2% 농도로 2시간 동안 유지시키고 복부 절개한 후 내장을 노출시켜 문지르고 superior mesenteric artery를 clipping 하는 자극을 주었다. 2시간의 마취가 끝난 뒤 2일째 해마 관련 인지기능 장애가 관찰되었다[7]. 다른 연구에서는 C57BL/6 수컷 마우스를 10% chloral hydrate (0.03 ml/10 g)로 복강 주사하여 마취시키고 0.25% bupivacaine을 내피 주사하여 상 복부에서 개복술을 하고 부분적인 간 절제술을 15분 동안 진행하였다. 이 결과 수술 후 3일과 7일째 공간기억장애가 나타났다[8]. 또한, Tian 등[9,10]의 연구에서, 간 절제술을 C57BL/6 암컷 마우스(14개월령)에서 시행하였는데 간의 왼쪽 측면엽(간의 약 30%)을 절제하였고 해마성 기억장애가 나타났다. Kamer 등[11]의 연구에서, Swiss Webster 수컷 마우스(6–8주령)를 3% isoflurane으로 마취시키고 2.5%로 유지시키고 난 후, 복부를 1 cm로 절개하여 비장 절제술을 실시하였다. 이 수술은 5분 동안 진행하였고 비장 절제술 이후, 단기 작업 기억 장애가 나타남을 확인하였다. 유사하게 Haile 등[12]은 Swiss Webster (6–8주령)를 2.5% isoflurane으로 마취시키고 1 cm 복부 절개하여 비장 절제술을 시행하였는데 무기력증 및 기억손상이 유발되었다.

Minor abdominal surgery model

임상에서의 복부 수술을 재현하기 위해서 C57BL/6 마우스(6개월령과 20개월령)를 chloral hydrate (5%)로 마취하고 복부 1.5 cm 절개하여 멸균 프루브로 소장, 간, 결장, 위의 차례로 1분간 장기를 자극하고 봉하였다. 수술 후 인지 기능 장애가 20개월령 마우스에서 나타났다[13]. Xu 등[14]의 연구에서, C57BL/6 암컷 마우스(9개월령과 18개월령)를 사용하여 복부 수술을 시행하였다. Bupivacine (0.5%, 0.1 ml) 마취 하에 복부를 2.5 cm의 길이로 절개 후 복강을 닫았다. 수술은 5분간 진행하였고 진정제는 사용하지 않았다. 수술 관련 통증을 완화하기 위하여 EMLA 크림(2.5% lidocaine 및 2.5% prilocaine) (Astra Pharmaceuticals, USA)을 수술 후 1일과 2일째 8시간마다 처치하였다. 그 결과 18개월령 마우스에서 인지기능 손상이 나타났다. Li 등[15]은 C57BL/6J 수컷 마우스(12–14개월령)를 수술모델로 사용하였으며 4% chloral hydrate (10 ml/kg)와 0.1% lidocaine으로 마취시켜 복부를 절개 후, 간, 비장, 신장 및 장을 포함하는 복부 장기를 면으로 30분 동안 자극하였다. 행동테스트를 통하여 인지손상을 확인하였다. Qiu 등[16]의 연구에서, C57BL/6 수컷 마우스(16개월령)를 1.5% isoflurane 마취하고 1 cm의 복부 절개 후, 내장 및 근육조직을 자극하였다. 이후 해마 의존성 기억장애를 유발하였다.

Tibial fracture surgery model

경골 골절술은 수술 후 인지 기능 장애 모델에 자주 행해지는 수술 방법 중 하나이다. Terrando 등[17]과 Cibelli 등[18]은 마우스를 isoflurane 2.1%와 buprenorphine (Buprenex, 0.1 mg/kg)를 피하 투여하여 마취시키고 왼쪽 뒷다리의 중앙을 절개하고 0.38 mm의 핀을 경골 골수관내 삽입하여 경골 골절술을 시행하였다. 두 연구 모두에서 수술 후 해마 의존성 기억장애가 관찰되었다[17,18]. 또한, 정형외과 수술 후 감염을 재현하기 위하여 C57BL/6 마우스에서 외과 수술 시행 2시간 전에 lipopolysaccharide (50 ng/kg)를 복강 투여하여 전신 염증을 일으키고 2.1% isoflurane과 buprenorphine 0.1 mg/kg 마취 하에 무균 상태에서 경골실험을 진행하였다. 수술 후 해마 의존성 기억손상이 유도되었다[19]. Shen 등[20]의 연구에서, C57BL/6J 수컷 마우스(12–14주령)를 1.2%–1.5% isoflurane에 마취 후 0.38 mm의 핀을 좌측 뒷발 절개 후 골수강 내로 핀을 삽입한 골수 내 고정술을 시행한 후 경골 골절을 유도하였다. 학습과 기억의 장애가 나타났다. C57BL/6 수컷 마우스(4개월령)을 1.5% isoflurane으로 마취하여 경골 노출 후 0.3 mm핀을 이용한 골수강 내 고정술을 이용한 경골 골절 수술을 시행한 실험에서, 수술 후에 해마 의존성 기억 손상이 나타났다[21].

전임상에서 인지기능 측정 방법

Morris water maze

Morris water maze는 주로 설치류의 행동을 평가하는 도구로 공간 학습과 기억을 연구하는 데 사용하기 위하여 고안된 장치이다[22]. 알츠하이머병, 파킨슨병, 헌팅턴병 등의 신경퇴행성 질환 모델에서 학습과 기억을 기전을 연구하고 뇌의 특정 영역에 대한 손상을 연구하는 행동테스트로 질환에 대한 신경 보호 및 치료의 후보물질이 학습과 기억에 미치는 영향을 평가할 수 있다[22-25]. 물로 채워진 원형탱크에서 실험동물이 시각적 단서를 이용하여 숨겨진/보이는 플랫폼을 탐색하는 과정을 관찰한다. 평가는 플랫폼까지의 수영 거리, 속도 및 탈출 대기 시간(latency time) 등을 분석하여 측정한다[9,26]. 수술 후 인지 기능 장애 모델에서도 적용되고 있으며 간 절제술과 경미한 복부 수술로 유도한 실험동물모델에서 플랫폼까지의 수영 거리 증가 및 탈출 대기 시간이 정상수준에 비하여 길어지는 특징이 나타났다[9,15,26,27].

Novel object recognition test

Novel object recognition test는 단기 작업 기억을 평가하는 행동학적 도구로 사용되고 있다. 다른 테스트와 비교하여 상대적으로 설치류에게 주는 스트레스가 적으며 효율적으로 기억력을 테스트할 수 있어 전임상에서는 유전자의 조작, 약리학적 조절 이후에 신경생리학적 변화의 검출이 용이하다[28]. 이 장치는 설치류의 새로운 대상을 탐구하는데 시간을 더 많이 할애하는 특징을 이용한 것으로 동일한 두 개의 물체를 실험동물에게 제시한 후 익숙하게 만들고 그 사물 중 하나를 새로운 대상으로 교체하여 노출할 시 실험동물이 익숙한 대상과 새로운 대상을 탐험하는 패턴을 관찰하는 방법이다. 단기 작업 기능 장애가 있는 경우에는 새로운 것과 익숙한 것을 구별하는 능력이 떨어지며 새로운 대상을 탐구하는 것에 시간을 소비하지 않는다[28]. 복부 대수술로 유도한 수술 후 인지 기능 장애 모델의 경우 새로운 대상을 탐구하는 시간이 정상 평균수준보다 줄어듦으로써 익숙한 대상과 새로운 대상의 탐색시간에 차이가 없게 되었다[7].

Passive avoidance task

Passive avoidance task는 수동회피장비로 학습과 기억을 측정하는데 사용하는 행동학적 도구이다. 이는 밝은 구획과 어두운 구획으로 나누어진 상자모양의 도구로 간단하고 신속한 것이 특징이며 신경정신질환의 인지 기능 평가 시 사용되고 있다[29]. 간단히, 실험동물을 밝은 구획에 두었을 때, 밝은 빛의 조명장치를 피해 어두운 구획으로 들어가고 바닥에 깔린 막대를 통해 전기 자극을 주어 기억을 심어준다. 이후 동일한 실험을 시행하였을 때, 밝은 곳에서 어두운 곳을 들어가면 전기 충격이 온다는 기억을 잘 형성한 실험동물의 경우 밝은 곳에서 머무르는 시간이 길다[7,30]. 복부 대수술을 통한 인지 기능 장애 모델의 경우 밝은 곳에서 어두운 곳으로 들어가는 시간이 상대적으로 정상군에 비해서 짧았으며 이는 기억능력이 감소됨을 확인하였다[30].

Elevated plus maze

Elevated plus maze는 불안 및 기억을 측정하기 위한 장치 중 하나로 우울증, 외상 후 스트레스 장애나 외상성 뇌손상 모델에서 많이 사용이 되고 있다[7,31-34]. 이 장치는 십자형으로 두 개의 개방된 팔과 두 개의 밀폐된 팔로 구성되어 설치류가 개방 공간에 대한 혐오감을 가지고 있다는 특징을 이용하여 행동을 분석하는 도구이다. 불안도 측정은 개방된 팔에서 실험동물이 소비하는 시간과 출입하는 횟수를 확인하고 기억 측정 위해서는 훈련기간에 밀폐된 팔과 개방된 팔을 탐험했던 기억으로 얼마나 빠르게 밀폐된 팔로 들어가는지의 시간을 측정하는 방법이다[7,34]. 복부 대수술을 통한 수술 후 인지 기능 장애 모델에서 기억 측정 패러다임을 적용하였을 때, 밀폐된 팔로 들어가는 시간이 정상상태의 범위보다 상대적으로 길었다[7].

T maze test

T maze test는 T자형 미로로 이루어진 동물 인지 능력 평가 방법이다[35]. 이 장치는 시작하는 팔과 두 개의 목표 팔로 구성되어 있으며 실험동물을 자유롭게 탐험하여 목표 팔을 선택할 수 있게 둔다. 그다음 시도에서는 최근의 선택을 기억하여 선택하지 않은 목표 팔을 선택한 경우, 즉 ‘자발적 교대’를 수행한 경우, 실험동물이 선호하는 음식 등의 보상을 주어 기억을 강화시킨다. 이후 탐험 패턴을 관찰하여 인지 기능을 판단한다. 이 자발적 교대의 과정은 해마, 시상, 소뇌 및 흑질 다양한 뇌 영역이 관여하는 것으로 알려져 있으며 특히, 프리온 질환에서 손상된 자발적 교대가 나타난다고 보고되고 있다[35]. 마찬가지로 간 절제술로 인한 수술 후 인지 기능 장애 모델에 적용한 경우에 이런 자발적 교대 및 보상 기억이 손상되는 경향을 보였다[10].

전임상에서 수술 후 인지 기능 장애 기전

신경염증(neuroinflammation)

수술 과정 및 수술 후 외상은 인지 기능 장애를 유발할 수 있는데 이는 인지 기능에 관여하는 뇌 영역의 염증반응과 관련이 있다는 보고들이 많다[7,11,30]. 학습과 기억에 중요한 역할을 하는 뇌 영역인 해마에서의 아교세포 활성화로 인한 interleukin-1beta (IL-1β), IL-6, 그리고 tumor necrosis factor alpha (TNF-α)와 같은 전 염증성 사이토카인 증가가 신경세포 사멸에 영향을 미치며 이런 일련의 과정은 인지 기능 손상에 영향을 미치는 것으로 보인다[7,36,37]. 또한, 신경염증은 신경발생 억제, 미세아교세포 priming (미세아교세포 활성화를 위한 임계값[threshold]의 변화)을 통해 인지 장애를 유발할 수 있다[38,39]. 전기생리학적 연구에서 시냅스의 기능 및 효율이 수술 후 인지 기능 장애 모델에서 현저히 저하되는 것으로 미루어보아 시냅스의 기능 결손 또한 인지 기능 장애에 주된 역할을 할 것이라 사료된다[7,30]. 말초에서의 수술 이후, 수술 부위에서의 조직 손상과 선천적 면역체계의 활성화로 전신 염증이 유발되고 혈류를 통한 뇌로의 염증성 인자의 유입이 인지 기능 장애에 영향을 미칠 수 있다[17,18].

혈뇌장벽 기능 장애(blood-brain barrier dysfunction)

혈뇌장벽의 투과성 증가와 같은 혈뇌장벽의 기능 장애는 인지 손상과 유의한 상관관계를 보여왔다[7,40]. 혈뇌장벽의 뇌로 수송된 염증성 사이토카인에 의해서 붕괴될 수 있으며 내피세포에서 분비하는 효소인 matrix metallopeptidases (MMPs)에 의해서 혈뇌장벽의 붕괴로 이어질 수 있다[7,41]. 염증성 매개체에 의해 활성화된 내피세포는 대식세포와 같은 말초세포를 포획할 수 있는 분자(vascular cell adhesion molecule-1, inter춃ellular adhesion molecule-1, p-selectin, 또는 e-selectin) 등을 내피세포 표면에 발현하여 중추신경계 내로의 유입을 용이하게 한다[7,42]. 또한, 붕괴된 혈뇌장벽을 통해 과도하게 침투한 호중구 및 대식세포는 염증 매개체를 생산함으로써 뇌 손상을 촉진시킬 수 있으며 동시에 뇌 전체 영역에 분산되어 있는 아교세포 활성화로 전염증성 인자의 증폭이 유도될 수 있다[7,43,44]. 마취 및 수술로 혈뇌장벽 투과성 증가될 수 있으며 학습력과 기억력 손상이 나타난다는 보고가 있었다. 이 현상에 중요한 역할을 하는 것이 MMP-9였으며 MMP-9이 결핍된 마우스에서의 수술은 혈뇌장벽의 투과성 및 인지 기능 손상에 영향을 미치지 않았다[41]. 정형외과 수술 이후 노령의 쥐에서도 해마에서 MMP-2와 MMP-9의 증가가 관찰되었고 혈관내피세포를 이어주는 tight junction 단백질인 occludin와 claudin-5가 감소되어 있었다. 이 변화는 혈뇌장벽의 투과성 증가로 이어졌으며 행동평가에서 인지 기능 손상의 소견이 관찰되었다[45].

Amyloid beta (Aβ)

Aβ는 알츠하이머병의 병리학적 표지자로 알려져 있다. 이는 아밀로이드 전구체 단백질(beta-amyloid precursor protein)에 의해 유도되고 응집된 형태의 용해성 올리고머를 형성/축적되어 amyloid plaque로 발전하게 된다. Amyloid plaque는 신경 세포 독성을 가지고 있으며 알츠하이머병을 유발할 수 있다[46,47]. 수술 후 인지 기능 장애 발생 후 알츠하이머병으로의 발병 가능성이 여러 차례 제시되어 왔다. 이는 Aβ라는 공통인자가 두 증상 모두에서 증가된 소견을 보였기 때문인데 수술 후 인지 기능 장애의 경우 부분 절제술이 Aβ 생산을 유도한다는 연구결과가 있었다[8,38,48]. 또한, 알츠하이머병에서 Aβ 주변부에서 염증반응이 주로 관찰되는데 이는 수술 후 인지 기능 장애의 병리학적 변화와 유사하다[8]. 전임상 연구에서 Aβ을 억제하기 위한 다양한 노력은 항체 처리법, small interfering RNA, 저해제 처리법을 통해 이루어져 왔다. 그 결과 Aβ 생산 감소는 인지 기능 장애 예방에 기여하였다[8,10,20,38].

Tau 과인산화(tau hyperphosphorylation)

Tau는 신경세포에서 주로 나타나는 미세소관 관련 단백질로 미세소관을 안정화시키고 축삭 돌기를 증진시키는 역할을 한다. Tau 단백질은 가용성으로 응집되는 경향이 거의 없으나 tau 단백질의 응집은 시냅스의 기능 손상 및 신경세포 사멸을 동반하여 taupathies라고 불리는 신경 퇴행성 질환을 유발할 수 있다[49,50]. tau의 인산화는 병리학적 상태 하에서, 미세소관의 결합력을 감소시킴으로써 세포뼈대를 불안정화시키며 neurofibrillary tangles (신경섬유엉킴)을 촉진시킨다. 인지 기능 저하 및 치매 증상을 보이는 알츠하이머병, 파킨슨병에서 tau 단백질은 공통적 병리학적 특성으로 보고 있으며 특히, 알츠하이머병에서의 tau와 Aβ의 병리학적으로 상호작용이 있음이 보고되었다[50,51]. 정형외과 수술 및 개복술 이후에 tau 단백질이 해마에서의 수준이 증가되었고 한 연구에서는 sevoflurane 고농도 노출이 tau 인산화와 관련성도 제시되었다[8,20,52]. Tau 단백질은 수술 후 인지 기능 장애의 기전 중 하나로 여겨진다.

전임상에서 수술 후 인지 기능 장애 예방 및 치료 후보물질

Erythropoietin (EPO)

EPO는 주로 신장에서 생산되어 혈액 순환계로 분비되는 당단백질로 알려져 있다[53]. EPO는 적혈구의 생산에 필요하며 적혈구 전구세포의 세포 사멸을 억제하는 역할을 한다[30]. EPO는 다양한 세포신호를 조절하여 신경생존에 관여하고 신경 전구 세포의 증식 및 분화에 관여한다고 보고 되고 있으며 면역 세포의 반응 감소, 염증성 사이토카인의 생산 억제 등의 염증 작용에 대해서도 보고되고 있다[53,54]. 이러한 보호 역할로 EPO는 알츠하이머병, 뇌전증, 파킨슨병 등의 운동신경 질환 및 신경퇴행성 질환을 예방하는 것으로 알려져 있다[54]. 이전 연구에서 노화 쥐에서 인지 손상이 나타났고 이는 해마에서의 EPO 발현의 감소와 관련이 있었다[55]. 또한, 알츠하이머병 마우스 모델에서 EPO는 신경퇴행성 진행의 중요 요인인 Aβ가 유도하는 인지 기능 장애와 tau의 인산화를 약화시켰으며 Aβ가 유도하는 세포 사멸을 억제하는 역할을 하였다[56]. 당뇨병 관련 인지 기능 이상에서 EPO의 처리는 기억 능력 결손을 예방하였다[57]. 수술 후 인지 기능 장애 모델의 경우 재조합 인간(rhEPO)의 처리는 해마에서의 신경염증을 감소시키고 시냅스의 기능 장애 및 인지 기능 장애를 감소시켰다. 또한, EPO가 대식세포의 표현형에 영향을 미침으로써 항염증 효과를 가져왔다. 단일 대식세포 배양 및 미세아교세포의 배양에서도 EPO의 처리가 염증성 사이토카인의 감소시키고 항염증성 사이토카인의 증가시키는 것을 확인하였다. EPO는 수술 후 항염증 작용을 함으로써 POCD를 예방할 수 있다는 연구결과가 제시되었다[30].

Nuclear factor kappa B (NF-κB) 억제제

전사인자로 알려진 NF-κB는 RelB, C-REL, NF-κB1 (p105/p50) 또는 NF-κB (p100/p52) 등의 다양한 동종 또는 이종의 하위단위로 구성되어 있다. NF-κB/REL 계열은 면역 반응을 조절하고 다양한 염증성 유전자의 발현을 조절하는데 핵심적인 역할을 하며 면역계 및 염증성질환 뿐만 아니라 암에서 관여하는 것으로 보고되고 있다[58-60]. 그리고 혈관성 치매 모델에서 별아교세포의 NF-κB가 백질의 병변과 기억력 결핍에 관여하며 노화 쥐에서 마취제 isoflurane으로 유도한 인지 기능 손상이 NF-κB와 관계있음이 보고되었다[61,62]. 특히 NF-κB p65는 패혈증 동물모델에서 신체 내 염증 반응에 관여하였으며 p65의 억제가 마우스의 혈장에서 전 염증성 사이토카인의 수준을 감소시킴으로써 전신염증을 줄여주었다[63]. 복부 수술로 유도된 수술 후 인지 기능 장애 모델에서 p65의 전사활동을 효과적으로 억제하는 합성단백질을 처리한 경우, 비장에서의 전염증인자의 감소 및 혈뇌장벽의 붕괴 감소로 이어졌다. 이는 대식세포의 뇌로의 유입을 막음과 동시에 해마에 존재하는 미세아교세포의 과활성 억제로 이어져 신경염증 유발을 감소시켰다. 그 결과 수술 후 나타나는 인지 기능 장애가 감소되었다[7]. 이를 통해 NF-κB p65의 억제가 수술 후 인지 기능 장애의 예방책으로 고려될 수 있다.

IL-17A 항체

IL-17A는 헬퍼 T세포(TH)17세포에 의해서 생성되는 사이토카인으로 자가면역 질환 및 염증성 질환과 연관성이 있음이 보고되어 왔다[10,64]. 또한, 중추신경계의 염증 관련 질병의 발병 기전에 IL-17A가 중요한 인자라는 보고가 이어져 오고 있어 질병의 치료방법 모색에 고려되어 왔다[65]. 이전의 연구에서 노화 쥐에 패혈증을 유도하였을 때, 해마에서 IL-17A가 및 기억손상을 확인하였고 이를 항체 처리법으로 억제 시켰을 때, 기억손상을 개선시키는 것을 확인하였다[66]. 간 절제술을 이용한 수술 후 인지 기능 장애 모델에서 수술 후 1일과 3일째 해마에서 IL-17A가 증가가 되었고 인지기능 장애, Aβ1–42, 그리고 별아교세포의 활성과 유의한 상관관계가 있었다. IL-17A 항체 처리로 기능 억제 시, 수술이 유도하는 인지 기능 장애 및 별아교세포 활성화와 Aβ의 축적을 억제시켰다[10].

TNF-α 항체

TNF-α는 전염증성 인자로 세포 분열, 분화, 사멸 등 다양한 생물학적 기능을 조절하는 데 관여하고 있다[17,67]. 혈장에서의 TNF receptor의 수준은 알츠하이머병, 파킨슨병의 바이오마커로 제시되어왔다[68,69]. 또한, 유전자 변형 알츠하이머병 어린 쥐에서 TNF-α 억제제 투여는 효과적으로 인지 기능 장애를 예방하였다[70]. 임상에서의 연구 결과에 따르면 청소년의 특발성 척추 측만증 수술 후 인지 기능 장애가 발생한 환자 혈장에서 TNF-α의 농도는 비 환자에 비해 유의한 상관관계가 있었다[71]. 전임상 연구에서는 개복술을 실시한 노화 쥐에서 인지 기능 장애가 발생했고 TNF-α 수용체 길항제의 투여는 IL-4, IL-6, IL-1β와 같은 전 염증성 인자를 감소시키고 인지 손상을 예방하였다[67]. 그뿐 아니라 경골 골절술을 시행한 마우스에서 TNF-α 항체 처리로 신경염증 및 수술 후 인지 손상을 예방하였다[17]. 전임상뿐만 아니라 임상에서도 보고되고 있는 TNF-α는 수술 후 인지 기능 장애의 예방과 치료제로 고려될 수 있다.

Nicotinamide adenine dinucleotide phosphate (NADPH) oxidase 2 (Nox2) 억제제

Nox2는 중추신경계에서 NADPH oxidase의 주요한 isoform 중 하나로 미세아교세포 등의 reactive oxygen species (ROS)의 과형성에 관여하며 NADPH oxidase는 미세아교세포의 활성과 TNF-α와 IL-1β 같은 다양한 전 염증성 사이토카인의 형성을 증폭시키는 데 중요한 역할을 한다[16,72]. 노화로 인한 뇌혈관 조절 장애에 superoxide 생성 효소인 NADPH oxidase가 관여한다는 보고가 있었고 Nox2가 결핍된 마우스에서 노화로 인한 ROS에 기인한 뇌혈관 손상이 관찰되지 않았다[73]. 이 혈관 산화 스트레스는 알츠하이머병의 기전과 관련이 있으며 유전자 변형 알츠하이머병 유발 쥐에서 Nox2의 결핍은 뇌혈관 기능 장애 및 인지 기능 장애가 관찰되지 않았다[74]. 개복술로 유도된 수술 후 인기 지능 장애 모델의 경우, 해마의 미세아교세포에서 Nox2 유래 ROS가 신경염증과 인지 기능 장애를 노화 쥐에서 유도하였다. 수술 후 7일째 해마에서 Nox2를 비롯한 염증 인자 및 DNA 산화 표지자가 증가되었고 NADPH oxidase inhibitor apocynin를 처리하였을 때, 수술로 유도되는 인지 기능 장애가 개선되고 관련된 뇌의 병태생리도 관찰되지 않았다[16]. 산화적 스트레스에 관여하는 Nox2의 억제는 수술 후 인지 기능 장애의 예방과 치료책으로 제시될 수 있다.

결론

수술 기법 발전, 마취 관리 및 집중 치료의 향상에도 불구하고 많은 환자들이 수술 후 인지 기능 장애를 겪고 있으며 이를 극복하는 것이 중요한 임상적 문제 중 하나이다. 이를 위해서는 먼저 그 기전/병태 생리학을 이해하는 것이 고려되어야 하며 효과적인 수술 후 인지 기능 장애의 예방과 치료책이 제시되어야 한다. 우리는 전임상에서의 수술 후 인지 기능 장애의 연구 결과에 중점을 두었고 전임상에서 제시한 병적 기전과 위험요인 및 예방/치료책을 알아봄으로써 수술 후 인지 기능 장애를 극복할 수 있는 발판을 제시하였다(Fig. 1). 지금도 여전히 전임상에서는 수술 후 인지 기능 장애를 해결하기 위한 연구와 약물 개발/발굴이 이루어지고 있으며 고찰을 통해서 인지 기능 장애의 문제점들을 하나씩 해결해 나갈 수 있길 기대한다.
